Sierra Carter has a diverse work experience in the healthcare industry. Sierra began their career at Aunt Martha's Youth Service Center as a Case Manager Assistant from June 2011 to September 2014. Sierra then joined Access Community Health Network as a Women's Health Coordinator from February 2015 to June 2016, where they conducted community outreach and educational workshops on breast and cervical cancer prevention.

Sierra then worked at Northwestern University - The Feinberg School of Medicine as a Case Manager/Research Assistant (Contract) from January 2017 to July 2017. In this role, they collaborated with returning citizens from the Cook County Department of Corrections, connecting them with essential resources for community reintegration. Sierra also leveraged data collection and analysis to identify process improvement opportunities and effectively communicated findings to interdisciplinary teams.

From February 2018 to January 2019, Sierra served as a Customer Service Representative at Evolent Health, where they addressed inquiries regarding health insurance benefits, eligibility, and claims. Sierra established effective relationships with physician office staff for streamlined communication.

Sierra then joined Advocate Aurora Health as a Central Scheduler & Patient Coverage Rep from November 2020 to October 2022. In this role, they managed scheduling of daily outpatient testing appointments across Advocate Health locations and verified physician orders and referrals to ensure accurate claim processing. Sierra also collaborated with the business office using CRM for patient-initiated insurance concerns.

Currently, Sierra is working at Somatus as a Patient Health Coordinator since March 2023. Sierra conducts initial screening assessments to align patients with appropriate care coordination programs, educates patients on preventive monitoring, and provides face-to-face outreach for patients with telephonic barriers.

Throughout their career, Sierra has demonstrated a strong commitment to improving community healthcare, data-driven decision-making, and effective communication with interdisciplinary teams.
