Tara Moore, MSW, CCM has a diverse work experience in the social work field. Tara currently works as a Transitional Care Navigator and Social Worker at Transitions LifeCare. Prior to this, they served as the President of the Board of Directors at the North Carolina Down Syndrome Alliance.

Tara Moore, MSW, CCM has a Master of Social Work (M.S.W.) degree in Community, Management, and Policy Practice from the University of North Carolina at Chapel Hill, which they obtained from 2011 to 2012. Prior to that, they completed their Bachelor's of Social Work degree at Appalachian State University from 2007 to 2011. Additionally, Tara holds the certification of Certified Care Manager (CMC) from the National Academy of Certified Care Managers, which they obtained in June 2021.
