JOB STATUS: Full time (40 hours/week)

LOCATION: Willmar, MN

RESPONSIBILITIES:
The Self-Sufficiency Case Manager’s responsibility is to empower individuals and families through case management facilitating the development of skills, abilities, confidence, and strength within people. To enable them to determine and pursue their own personal goals and to strive to help them reach a level where they are able to continue moving forward on their own. To assist homeless individuals and families in stabilizing their housing situations. To assist Self-Sufficiency participants in becoming independent of public assistance.

EXAMPLES OF DUTIES:
• Empower individuals and families through case management facilitating the development of skills,
abilities, confidence, and strength within people.
• Assist homeless individuals and families in stabilizing their housing situations.
• Assist participants in becoming independent of public assistance.
• Conduct meetings and assessments and develop individualized plans with participants.
• Provide enrollment, assessment, goal development, and program services with participants.
• Provide participants with ongoing case management and support.
• Implement and coordinate the Self-Sufficiency training as directed.
• Complete record keeping, case notes, data collection, and other documentation.
• Establish and maintain an effective referral network and community linkages.
• Attend conferences, meetings, and training as needed or requested.

EDUCATION AND EXPERIENCE:
• Bachelor’s degree in related field and two (2) years related work experience OR combination of six
(6) years of related education and experience.

• Demonstrated effective written, oral, and interpersonal communication skills.
• Awareness of and sensitivity toward economic and cultural issues facing those we serve.
• Ability to promote, support, advocate for, and facilitate change in individuals.
• Ability to provide assistance to individuals and families in crisis situations.
• Understand and respect individual choices and diversity.
• Ability to facilitate comprehensive assessments with participants.
• Ability to set, monitor, and evaluate participant goals.
• Ability to work independently and cooperatively as part of a team.
• Demonstrated skills in problem solving and conflict resolution.
• Ability to travel for meetings, workshops, and conferences.
• Reliable mode of transportation.
• Reliable, regular attendance.
• Completion of a satisfactory background check
