Population Health Manager at Unity Care NW

Nicole Fields, DHSc, MPH, CHES has a strong background in public health and program management. Nicole currently works as the Population Health Manager at Unity Care NW since August 2020. Prior to this, Nicole worked at Lincoln County from September 2018 to December 2021, where they held the positions of Deputy Director- Public Health and Health Promotion Program Manager/Accreditation Coordinator. In these roles, they developed program objectives, collaborated with various stakeholders, prepared annual plans and budgets, and supervised health promotion programs. From March 2014 to March 2016, they worked as a Health Education Specialist at Linn County Public Health, focusing on injury, violence, ACEs, mental health, and substance abuse. Before entering the public health field, Nicole gained experience as a Housing Inspector at PROPERTY MANAGEMENT SERVICES from July 2013 to April 2014. Nicole also worked as a Shift Supervisor/Manager/Server at RAM for a significant period from March 2004 to July 2013, where they provided excellent customer service and supervised staff. Other notable experience includes working as a Tutor at Tutor.com from December 2011 to November 2012 and as an Environmental Educator at Garfield Park Conservatory from May 2011 to September 2011. Nicole brings a diverse range of skills and expertise to their work in the public health sector.

Nicole Fields, DHSc, MPH, CHES, has an extensive education history. Nicole is currently pursuing a Doctor of Health Science (DHSc) degree at A.T. Still University of Health Sciences, which they plan to complete in 2022. Prior to this, Nicole obtained a Master's Degree in Public Health from Benedictine University. Their undergraduate studies were completed at Northeastern Illinois University, where they earned a Bachelor of Arts (B.A.) in Environmental Studies.
