POSITION PURPOSE:
Under the general supervision of the RN Case Manager, the Care Coordinator performs certain case management and clinical duties. Case management duties may include transitions of care, chronic care management, Coumadin management, Alternative Payment Care Model-related (APCM-related) activities, and other case management duties. The Care Coordinator promotes and evaluates the effective utilization of resources using current knowledge and awareness of community resources and assumes a pivotal role with the patient care team to achieve optimal clinical and resource outcomes. Clinical duties may include taking and recording vital signs, drawing certain labs, and direct care duties as needed and in accordance with established policies and procedures. Administrative duties may include generalized care coordination, scheduling medical visits as indicated, and working with care teams to perform comprehensive chart reconciliation and review.
ESSENTIAL FUNCTIONS:
QUALIFICATIONS:
At least one (1) years’ experience working as part of a care management team managing complex patients, including transitions of care.
Experience in Care Coordination in Hospital or Clinical setting.
Previous experience as a Medical Assistant preferred, ability to obtain Medical Assistant certification preferred.
Current BLS certification (maintaining current certification is required for continued employment Aviva Health).
Ability to work independently, detail oriented, organized, work effectively under pressure, and meet deadlines.
Excellent computer and organizational skills.
Demonstrated knowledge of office procedures, patient flow, triage and scheduling.
Ability to communicate effectively with others using the spoken word and writing clearly and concisely. Ability to follow verbal and written directions.
Ability to accept responsibility and account for his/her actions and deliver upon expectations.
Knowledge of basic medical terminology.
These skills and abilities are typically required through a combination of experience and training.
