FUHN is a 501(c)3 nonprofit of 12 federally qualified health clinics (FQHC), predominantly based in the Twin Cities and Mankato metro areas. FQHCs are community-based organizations that provide comprehensive primary care and preventive care, including medical, dental, and mental health/substance abuse services to people of all ages and backgrounds, regardless of their ability to pay or health insurance status. FQHCs are a critical component of the U.S. healthcare safety net.

In 2012, 10 founding clinics formed an Integrated Health Partnership (IHP) with the Minnesota Department of Human Services on Medicaid healthcare reform efforts. Through the initial arrangement within the IHP, the participating clinics achieved more than $26 million in savings for the state. Today, these clinics aim to enhance the care delivered to Medical Assistance patients by coupling primary care with expanded use of technology and improved outreach.

​FUHN also serves as a Health Center Controlled Network (HCCN), funded by the Health Resources & Services Administration to strengthen health information technology (IT) to improve operational and clinical practices that result in better health outcomes. FUHN supports its member clinics by leveraging health IT and data to deliver high-quality comprehensive primary healthcare and effectively participate in value-based care arrangements.

The FUHN team focuses on improving clinical quality, patient-centered care, and provider and staff well-being. Together FUHN and the participating clinics have a shared vision for delivering high-quality healthcare for people in the Twin Cities and across Minnesota.
