Form Health is a virtual obesity medicine clinic delivering multi-disciplinary evidence-based obesity treatment through telemedicine. Obesity impacts more than 40% of the US adult population, and although historically only about 1% of patients received medical treatment for their disease, the field of Obesity Medicine is entering a period of rapid growth. Form Health provides high-quality expert care and leverages technology to enhance the patient experience. All Form Health patients work closely with their care team, which includes board certified physicians, advanced practice professionals and Registered Dietitians. Through our proprietary mobile app patients engage in regular video visits, as well as text messaging, photo journaling, digital data transmission, and customized educational materials. We hold ourselves to the highest standards of clinical care, and to treating every individual with empathy and respect.

About the Role:

We are seeking an experienced Human Resources leader to assume responsibility for directing and overseeing our company’s HR plans. This includes talent acquisition, compensation & benefits, talent management/relations, employee engagement, and learning & development.  We seek someone passionate about supporting the overall wellness of our employees - mental, physical and financial - and the overall health, adaptability and resilience of our organization. This is a full time opportunity reporting to our Chief Operating Officer. Work is remote but must be able and willing to travel 25% of the time. 

We seek applicants who have: 

  • A Master’s in Organizational Development, MBA, or equivalent experience.
  • 10 years management experience, 2 years at the executive level
  • Grown an HR team from scratch in a fast-paced, rapidly changing & growing environment
  • Supported an organization’s growth from 100 employees to 1000 employees
  • Used data-driven process to ensure their organization has the right people, in the right place, at the right time to achieve its strategic goals
  • Created a strong employment brand to attract talent
  • Created performance management programs that include clear goal-setting; regular, open feedback; and calibration 
  • Experience creating a learning & development program to enable employees to develop the skills they will need to best meet company needs not only now, but in the future
  • Created career paths, led the process of creating career ladders,  leveling employees appropriately, and ensuring learning & development supports employees along their career path
  • Developed programs to recognize and reward employees
  • Led significant projects on time and on budget. For example, selecting and replacing an HRIS. 
  • Exceptional communication and organizational skills; especially as it pertains to employee relations and change management

Base Annual Salary
$190,000$230,000 USD
