Overview 

HOPE Community Services, Inc. is a leading outpatient treatment provider in south Oklahoma City.  We provide compassionate trauma informed care to individuals whose lives have been impacted by mental health and substance use disorders. We do this by providing compassionate and evidence-based treatment, aimed at improving the quality of life for those we serve. 

We are currently in search of a Care Coordinator. This individual will work closely with children, youth, and families, facilitating services and connecting them to vital community resources. Do you have experience working with children and families in a mental health or social services setting? Are you passionate about advocating for families and coordinating care to help them succeed? About the Position

As a Care Coordinator, you will have the opportunity to make a meaningful impact in the lives of children, youth, and families by providing essential support and guidance. In this role, you’ll facilitate access to services and community resources, helping families access the care they need for long-term stability. You’ll play a critical part in ensuring children and youth can remain in their homes and schools, while also building valuable relationships with community partners and service providers. This role offers both personal fulfillment and professional growth as you support families in achieving their goals.

Requirements

  • Ability to obtain and maintain Case Management II (CM II) certification.

Skills

  • Detail-oriented with strong analytical skills.

  • Strong verbal and written communication skills.

  • Empathy and a commitment to maintaining a high standard of care.

  • Strong organizational skills, with the ability to prioritize tasks and manage multiple projects simultaneously.
