Sustaining Tenancy Navigator 

We are looking for one person who will be on a fixed term contract. 

Position Description

As a Community Housing Support Worker, you will uncover and unleash the strengths of the people you are working with by walking alongside to support them find suitable housing or sustain their current permanent housing situation.

You are responsible for coordinating a programme of planned holistic support, to people with a wide variety of needs, to build their skills and an appropriate network of support. The specialist support you provide not only makes a difference to their life now, but also provides building blocks to help them develop the skills and confidence to be self-supporting in the future.

You will use your interpersonal skills to form positive and professional partnerships with internal and external stakeholders including MSD, MHUD, social housing providers, Kainga Ora, landlords, moteliers and building managers.

You will provide and oversee connection to community based support services that are culturally responsive leading to the maintenance of sustainable long term housing, and positive wider social outcomes through whanau being connected to their community.

  • You have no direct reports

WHAT YOU’RE RESPONSIBLE FOR

You will work in a mana enhancing way with a mixed caseload:

  • Support with daily tasks and interventions so that people may learn, or improve, their practical life skills (e.g. budgeting, paying bills, planning meals etc.) needed for their sustainment in independent housing.
  • Support whanau to move to sustainable permanent housing and provide ongoing support if required
  • Broker and provide supported referrals to housing suppliers and other services as the need is identified
  • Create a strengths based Support and Action Plan
  • Providing information and advice on welfare rights, tenancy rights and responsibilities, and supporting people to develop self-advocacy and assertiveness skills in these areas.
  • Maintain relevant, timely and accurate documentation at all times
  • Developing an effective working relationship providing motivation and constructive challenge
  • Developing and implementing strategies to sustain their accommodation,
  • Delivering a service which is sensitive to the needs of diverse populations, is non-judgmental in approach, is accessible to all people and does not discriminate on grounds of age, class, disability, religion, ethnicity, gender, sexual orientation, HIV status or offending background in line with the Human Rights Act 1993
  • Deliver culturally responsive services, which incorporate peoples spiritual, cultural and holistic needs, that are whanau led, strengths based, and align with the principles of Te Tiriti o Waitangi.

WHAT SKILLS & EXPERIENCE YOU NEED

  • Ability to work collaboratively with your team
  • Proven ability to manage a mixed caseload, preferably in a community based setting
  • Positivity, flexibility, confidence and compassion with a non-judgmental attitude with clients and colleagues
  • Resilience, with the ability to work in an environment of unpredictability, and remain practical and focused
  • Have experience in working alongside people with mental health issues, substance abuse and/or trauma
  • Excellent interpersonal and communication skills (verbal, written and computer literacy)
  • Knowledge or understanding of motivational interviewing, harm reduction and or trauma informed care
  • The ability to understand, communicate with and effectively interact with people across cultures, demonstrating an awareness of your own world view and your positive attitudes towards cultural
  • Understanding of the principles of the Treaty of Waitangi and how they can be applied in practice
  • Relevant qualification or experience (including lived experience) in one of the following fields: social work, mental health, social housing, addictions, support work, peer or consumer advisor; with an ability to demonstrate practical transferable skills
  • Ability to make whanau led Support and Action plans ensuring that goals are SMART (Specific, Measurable, Achievable, Realistic and Time Bound), regularly reviewed and signed off when completed
  • Ability to work to deadlines, achieve results and be accountable for using your autonomy to ensure that the individual’s and the contracts goals are met
