One Community Health is a primary healthcare and specialty care provider dedicated to improving the health and well-being of our community.

Since our beginning in 1989, it has been our mission to create a healthier Sacramento by increasing access to care for all members of our community, regardless of age, gender, ethnicity, orientation, or ability to pay.

We want you to feel comfortable and well cared for. But more than that, we want you to be a partner in your care. We want you to understand, to ask questions, and to be aware that you can work in partnership with the staff to make life choices that will lead to your improved health.

We use a medical home model of care, including preventive care, routine checkups, immunizations, and management of acute and chronic illnesses. The patient-centered medical home—one of modern healthcare’s most important innovations—is a model of care that emphasizes care coordination and communication to transform primary care into “what you want it to be.”

The Healthcare and Medical Support team at One Community Health Sacramento is dedicated to providing comprehensive and coordinated care to patients. This multidisciplinary team includes medical and behavioral health clinicians, medical assistants, case managers, medical scribes, and quality improvement coordinators. Together, they ensure patients receive preventive care, routine checkups, and management of both acute and chronic conditions while maintaining high standards of quality and patient-centered care. Their collaborative efforts focus on improving health outcomes and increasing access to essential healthcare services for the Sacramento community.
