Our Intake team performs intake procedures including (but not limited to) identifying & evaluating patients referred for services, assisting with the authorization & caseload building process, and communicating with occupational, physical, and speech therapists regarding their Home Health clients.
Duties & Responsibilities:
- Serve as the primary point of contact for patient families regarding staffing schedules.
- Maintain compliance with federal and state regulations as they pertain to the agency.
- Work closely with office staff to promote positive and effective communication to ensure appropriate client care.
- Set and model quality customer service standards.
- Proactively follow up with families while managing and maintaining staffing schedules.
- Assume responsibility in coordinating care for assigned patients, working to support a goal-directed care plan from admission to discharge.
- Assist with answering incoming telephone calls from caregivers and clients.
- Collaborate with the Client Service Manager to maintain client relations and address service failures.
- Demonstrate the ability to prioritize multiple tasks to meet deadlines.
- Interact in a collaborative manner with other departments and teams.
- Identify and solve problems, using initiative and good judgment to reach quality decisions.
- Adhere to and demonstrate thorough knowledge of company policies and procedures.
- Maintain an excellent rapport with patients and staff, effectively promoting harmonious interpersonal relationships.
- Help gather caseload information to assist with the orientation process for new therapists.
