Care Management Team

The Care Management Team at Advance Care Alliance of New York is dedicated to coordinating and overseeing care for individuals with intellectual and developmental disabilities, ensuring they receive comprehensive support and services. The team members, including care managers, directors, and assistant directors, work collaboratively to develop personalized care plans that facilitate active, healthy, and fulfilling lives for the community they serve in New York City, Long Island, and the Hudson Valley.
