Care Coordinator at Boston Health Care for the Homeless Program

Anthony Wiafe's work experience includes serving as a Care Coordinator at the Boston Health Care for the Homeless Program, where they are responsible for coordinating care for up to 50 homeless individuals with serious mental illness, substance use disorders, and/or complex medical illnesses.
