Dani Santiago-Roach is a Social Determinants of Health Manager at Cooper University Health Care. Dani manages a team of Community Health Workers, Community Health Navigators, and SDOH projects/initiatives. Dani also oversees Cooper's program to address social determinants of health in vulnerable communities, collaborates for care coordination of at-risk populations, reduces hospital readmission rates for ACO patients, conducts HIV prevention education and testing, provides secretarial support for pediatric providers, conducts newborn hearing screenings, and has a background in Public Health Education and Promotion. Currently pursuing a Master of Business Administration degree.
