Gabriela M.

Gabriela M. has a diverse work experience spanning several industries. Gabriela currently serves as the Director of Care Management at Advance Care Alliance, a position they have held since October 2018. Prior to this, Gabriela worked at The THRIVE Network, where they held multiple roles including Medicaid Service Coordinator Supervisor and QIDP (Qualified Intellectual Disabilities Professional). Their responsibilities included developing and maintaining individualized service plans, coordinating valued outcomes for individuals with intellectual disabilities, and providing training to residential staff. Before their time at The THRIVE Network, Gabriela worked at PSCH as an Eligibility and Entitlements Specialist, where they acted as a liaison between clients and Medicaid and Social Security to ensure appropriate benefits. Gabriela also conducted screenings and interviews for prospective clients. Gabriela's earlier experience includes roles as a Program Aide at Learn It Systems, Housing Advocate: Entitlement at Catholic Charities Brooklyn and Queens, Computer Lab Assistant at Stony Brook University, and Clerical Assistant at New York State Nurses Assoc.

Gabriela M. attended Stony Brook University from 2005 to 2009, where they obtained a Bachelor of Science (B.S.) degree in Health/Health Care Administration/Management.
